What To Bring
We ask that you bring any past radiographic studies and their reports relevant to your child’s consultation. These studies may include, but are not limited to, chest x-rays, chest CT scans, swallow evaluation, and upper GI series. In addition, bring appropriate surgical reports, sleep study reports, laboratory reports, gastroesophageal study reports, or other test results. Please hand carry all films or disc (a CD is preferable so that we may view all images). We also ask that you please bring an updated list of your child’s current medications and bring your child’s asthma medications and spacer (aerochamber) device to your child’s initial consultation and all subsequent follow up visits.
What Dr. Shah Will Do
Dr. Shah will conduct a thorough consultation and will listen to your concerns about your child and his/her symptoms and answer all your questions. Based on your child’s symptoms, medical history, previous response to treatment, and other important characteristics he may order diagnostic testing to help better determine your child’s potential diagnosis and best treatment plan for your child. Examples of diagnostic studies include pulmonary function testing, chest x-ray, allergy testing, flexible bronchoscopy, swallow evaluation, chest CT scan, and sleep study. In collaboration with you, he will design an individualized treatment plan to address your child’s specific needs. Dr. Shah will work closely with you, your child’s primary care provider, and other pediatric specialists in order to offer his patients the most complete care.
Preparing For Your Child’s Visit
Children’s Asthma and Lung Specialists is conveniently located on Route 27 in Kendall Park. Dr. Shah’s medical office suite is located in the rear end of building 3086. For your convenience, there are three reserved parking spaces with additional parking nearby. With the office being in the rear of the building, there is not much car traffic. However, we do advise you to use precaution and keep a close eye on your child at all times.
Please recognize that it is important that you arrive at least 15 minutes prior to your scheduled appointment time so that all necessary paperwork can be completed and processed so that we can see your child as scheduled. In addition, this extra time allows you to get acquainted with our dedicated and compassionate staff.
